What does "case-by-case" mean here?

The lender doesn't publish a firm yes/no for your situation — an underwriter decides individually, usually based on income, deposit size or how long you've had the visa. It's not a rejection, but it's not a guaranteed yes either.

Why do some lenders need a minimum time left on my visa?

Lenders want assurance you'll still have the right to work and reside in the UK well into the mortgage term (or at least past the initial product period). A visa with only a few months left makes that harder to demonstrate, even if you intend to renew it.
